Tuition: $175(U.S. Dollars) per one day seminar
$325(U.S. Dollars) Seminars #202 & #290 (2 day seminars)
Registration form must be accompanied by your deposit check of $75.00 per seminar.
Balance due on or before first day of the seminar. 10% Discount per
seminar if you register before May 15, 2008.
Checks must be drawn on a U.S. Bank in U.S. Dollars made payable
to the Play Therapy Training Institute, or charge to your __ VISA or
__ Mastercard. Expiration date, CVV code (the last 3 digits of the number that
appears in the signature box on the back of your card) and signature are required with credit card payment.

CANCELLATION POLICY
To cancel your seminar reservation, we must receive notice NO LATER than June 30, 2008. All cancellations and changes will be subject to a $30.00 administrative charge per seminar. No refunds will be made after July 5, 2008.
